Precision Therapeutics for β-Sarcoglycanopathy

Bridging human molecular genetics, sarcolemmal bio-nanomechanics, and engineered cytoprotective peptide scaffolds to arrest disease progression in Limb-Girdle Muscular Dystrophy R4 (LGMD 2E).

Molecular Etiology

The "House of Cards" Structural Defect

01

The Nucleating Core

β-sarcoglycan (35 kDa) is the indispensable anchor of the tetrameric complex (α, β, γ, δ). If β-sarcoglycan is missing or misfolded, the remaining subunits cannot incorporate and undergo ER-associated degradation (ERAD).

02

Contraction Tears & Ca²⁺

Without the structural shock absorber, routine muscle contractions create sarcolemmal micro-ruptures. Extracellular calcium floods in, activating μ- and m-calpains that enzymatically digest internal titin and troponin.

03

Cardiac & Fibro-Fatty Threat

Chronic NF-κB inflammation triggers fibro-adipogenic progenitors (FAPs), turning functional muscle into scar tissue. High expression in cardiomyocytes also drives significant lifetime risk of dilated cardiomyopathy.

Genomic Variant Mapping

Compound Heterozygous SGCB Profile

Targeting the exact molecular defects identified in Class 5 pathogenic sequencing (RefSeq: NM_000232.5).

Allele 1 • Exon 3 Missense

c.341C>T | p.(Ser114Phe)

Serine replaced by bulky phenylalanine disrupts ectodomain folding. The protein is produced but trapped in the ER and routed to ERAD.

Intervention Target: Pharmacological chaperones & proteostasis regulators to rescue trafficking to the sarcolemma.
Allele 2 • Exon 4 Nonsense (Stop)

c.552T>G | p.(Tyr184*)

Introduction of a premature termination codon (PTC) at residue 184 causes nonsense-mediated mRNA decay (NMD) and zero functional translation.

Intervention Target: Translational read-through peptides / small molecules to bypass the stop codon.

Translational Chemistry

The 4-Layer Cytoprotective Shield

Targeted bio-nanomechanics engineered to seal membrane ruptures, quench calpain proteolysis, arrest fibrotic signaling, and preserve cardiac mitochondria.

Layer 1 • Membrane Defense t½: 5–7 Days (HSA)

Sarcolemmal Sealant-C20

Amphipathic α-helix that inserts into high-curvature contraction-induced lipid tears, acting as a molecular plug to halt extracellular Ca²⁺ influx. Appended with icosanedioic acid ([AEEA]2-γGlu-C20 diacid) for sustained albumin hitchhiking.

Ac-FKALKELLKAALELLKK([AEEA]2-γGlu-C20 diacid)-AALKAL-NH2
Layer 2 • Anti-Fibrotic Shield t½: 3–4 Days (HSA)

Stapled NBD-C18 Peptide

Hydrocarbon-stapled NEMO-binding domain with (S)-2-(4'-pentenyl)alanine crosslinked via ring-closing metathesis (RCM) at positions X1 (pos 5) and X2 (pos 9). Appended with octadecanedioic acid to block NF-κB/FAP fibro-adipogenic macrophage scarring.

Ac-T-A-L-D-X1-S-W-L-X2-T-E-K([AEEA]2-γGlu-C18 diacid)-NH2 [S5 staple]
Layer 3 • Protease Inhibition t½: 16–24 Hours

Cyclic Calpastatin Subdomain 14-mer

Head-to-tail amide backbone cyclic pseudo-substrate inhibitor that selectively shields sarcomeric titin and troponin from hyperactivated μ-calpain without proteasome cross-toxicity or non-specific off-target cleavage.

cyclo(Asp-Pro-Met-Ser-Thr-Ile-Tyr-Ile-Glu-Glu-Leu-Gly-Lys-Lys)
Layer 4 • Bioenergetic & Cardiac Daily SC Morning

SS-31 / Elamipretide Analogue

Mitochondria-targeted aromatic-cationic tetrapeptide that selectively partitions into inner mitochondrial cardiolipin, preserving electron transport chain cristae architecture, preventing mPTP opening, and defending against dilated cardiomyopathy.

D-Arg-2',6'-Dmt-Lys-Phe-NH2

Clinical Pharmacology

Synchronized Administration Regimen

Coordinated pharmacokinetics aligning sustained albumin-bound sarcolemmal protection with daily mitochondrial energetics.

Twice-Weekly Shield Depot Subcutaneous (SC) • q3.5d

Multi-Tier Membrane & Calpain Shield

Co-administered as a combined formulation in sterile, isotonic buffered saline (pH 7.4) twice per week:

  • 1. Entity 1 (Sealant-C20): Sarcolemmal tear plug (t½ ~5–7d albumin depot)
  • 2. Entity 2 (Stapled NBD-C18): NEMO uncoupler (t½ ~3–4d albumin depot)
  • 3. Entity 3 (Cyclic Calpastatin): Direct μ-calpain pseudo-substrate

Maintains continuous therapeutic serum concentrations above the critical micelle/resealing concentration ($C_{\text{seal}}$) across routine physical locomotion.

Daily Maintenance Regimen Subcutaneous (SC) • Every Morning

Mitochondrial & Cardiomyocyte Protection

Administered every morning upon waking to provide uninterrupted bioenergetic support:

Entity 4 (SS-31 / Elamipretide Analogue)
Dose: 0.25 – 0.50 mg/kg SC morning bolus

Rapidly crosses cell membranes and targets cardiomyocyte cardiolipin to ensure stable ATP generation during daytime metabolic demand and suppress reactive oxygen species (ROS) leakage.

CRITICAL THERAPEUTIC CONTRAINDICATION: ActRIIB / Myostatin Inhibitors

ActRIIB receptor decoys and myostatin inhibitors (e.g., bimagrumab, taldefgrobep alfa) as well as unshielded eccentric resistance loading are strictly contraindicated in unstabilized β-sarcoglycanopathy.

Driving myofiber hypertrophy or high-tension contractions while the sarcolemma lacks mechanical integrity induces massive membrane shearing, catastrophic extracellular Ca²⁺ influx, unconstrained calpain hyperactivation, and accelerated rhabdomyolysis. Sarcolemmal resealing must be biochemically confirmed via serum CK/myoglobin reductions prior to introducing any anabolic stimulus.
