Gene Therapy & CRISPR Horizons

Evaluating the clinical data for recombinant AAV-mediated cDNA replacement, vector-related immunological hurdles, and the emerging frontier of in vivo base and prime editing.

Recombinant AAV Replacement

The SRP-9003 (Bidridistrogene Xeboparvovec) Paradigm

The most clinically mature therapeutic strategy for LGMD R4 (LGMD2E) is adeno-associated viral (AAV) gene transfer, designed to deliver a full-length wild-type human SGCB coding sequence directly into skeletal and cardiac muscle fibers.

Vector Architecture: AAVrh74.MHCK7.hSGCB

Capsid: AAVrh74

Derived from non-human primates; displays strong tropism for both skeletal striated muscle and deep cardiomyocytes with lower baseline seroprevalence in human pediatric populations than AAV9 or AAV2.

Promoter: MHCK7

A hybrid murine creatine kinase promoter/enhancer with a cardiac regulatory element, restricting transgene expression selectively to striated muscle and heart while minimizing non-target liver transcription.

Transgene: Codon-Optimized hSGCB

Full-length 318 aa human β-sarcoglycan cDNA (35 kDa) configured without truncations, fully competent to nucleate the assembly of endogenous α-, γ-, and δ-sarcoglycans at the sarcolemma.

Clinical Trial Evidence Summary
Phase 3 EMERGENE (NCT06246513) Primary Endpoint Met

Evaluating a single systemic IV infusion of 7.41 ร— 10ยนยณ vg/kg in 11 ambulatory (Cohort 1) and 6 non-ambulatory (Cohort 2) patients. Day 60 biopsy results demonstrated statistically significant robust β-sarcoglycan reconstitution:

  • Ambulatory: Mean change of +43.4% in percent positive fibers (PPF, P < .001).
  • Non-Ambulatory: Mean change of +23.9% PPF (P < .02).
  • Western blot and fiber intensity confirm trans-sarcolemmal localization.
Phase 1/2 Long-Term Cohort (NCT03652259) 4-5 Year Durability

Children dosed in initial dose-escalation cohorts demonstrated durable expression and sustained motor function preservation up to 5 years post-infusion:

  • Sustained stabilization on the North Star Assessment for Dysferlinopathy (NSAD).
  • Persistent >70% reduction in serum Creatine Kinase (CK).
  • Safety profile consistent with expected manageable AAV immunogenicity.
Key Biological Boundary: The Single-Dose Ceiling & Capsid Immunity Systemic infusion of recombinant AAV induces high-titer neutralising antibodies (anti-AAVrh74 NAbs). Patients who have received an AAV dose currently cannot be redosed with the same capsid. Pre-existing antibodies also exclude a fraction of patients from initial trial eligibility. High-dose vector load requires rigorous steroid prophylaxis to prevent transaminitis and complement activation.

Genome Editing Horizons

CRISPR-Cas9, Base Editing & Prime Editing Feasibility

Unlike gene addition (which introduces an extrachromosomal episome), precision gene editing aims to permanently correct the native genomic DNA at the chromosomal locus in patient satellite stem cells and myofibers.

Target: c.341C>T (p.Ser114Phe) Transition Mutation

Adenine Base Editing (ABE): Scarless Repair Without DSBs

The c.341C>T missense mutation in exon 3 is a transition ($C \cdot G \to T \cdot A$). On the antisense strand, the pathogenic nucleotide is Adenine.

Targeting Mechanism: Cas9 Nickase (D10A) + Deoxyadenosine Deaminase (ABE8e) 1. sgRNA binds target genomic locus on Exon 3. 2. Deaminase converts antisense A → Inosine (I). 3. DNA polymerase treats Inosine as Guanine during repair/replication. 4. Outcome: Permanent reversion to wild-type C·G pair.

Major Safety Advantage: ABE avoids double-strand DNA breaks (DSBs), completely eliminating the risk of random indels, large chromosomal deletions, or p53-mediated apoptosis.

Status: Pre-clinical proof-of-concept in academic neuromuscular models.
Target: c.552T>G (p.Tyr184*) Transversion Mutation

Prime Editing (PE): Search-and-Replace PTC Overwrite

The c.552T>G nonsense mutation creates a premature stop codon (UAA). Classical base editors cannot perform transversion corrections ($T \to G$) cleanly.

Targeting Mechanism: Cas9 Nickase + Reverse Transcriptase + pegRNA 1. Prime editing guide RNA (pegRNA) specifies target and contains repair template. 2. Reverse transcriptase directly synthesizes corrected DNA strand into nick. 3. Overwrites premature stop codon UAA → Tyrosine triplet (UAC / UAU). 4. Outcome: Restores full-length translation and eliminates NMD decay.

Translational Challenge: Prime editors are large (~6.5 kb), exceeding standard single-AAV packaging capacity (~4.7 kb), requiring dual split-intein AAVs or non-viral delivery.

Status: Laboratory optimization for neuromuscular diseases.

Next-Generation In Vivo Delivery: Overcoming AAV Limits with LNPs & eVLPs

The ultimate success of CRISPR and base editing in monogenic myopathies depends on delivery. Because permanent expression of bacterial Cas9 protein causes immune rejection and off-target cleavage, the field is pivoting toward transient non-viral delivery:

Selective Organ Targeting (SORT) LNPs

Lipid nanoparticles engineered with supplemental biophysical lipids that redirect biodistribution away from the liver and directly to skeletal muscle and cardiac myocytes. Can deliver mRNA encoding base editors for short 24โ€“48 hour pulses.

Engineered Virus-Like Particles (eVLPs)

Protein capsids that deliver pre-assembled Cas9/base editor ribonucleoprotein (RNP) complexes directly into cells without carrying viral DNA. Zero genomic integration risk and fully compatible with redosing.
