Clinical & Translational Governance Matrix
Validated Clinical Endpoints & Exploratory Biomarkers
Integrating natural history outcome assessments established by the GRASP-LGMD Consortium (Doody, Alfano, Diaz-Manera et al. BMC Neurol 2024, PMID: 38491364) and clinical gene therapy trials (PMID: 38177855) alongside exploratory translational safety markers.
| Biomarker / Modality | Evidence Status | Assessment Cadence | Clinical & Biological Interpretation |
|---|---|---|---|
|
Functional Motor Batteries
NSAD, 10M Walk/Run, 4-Stair Climb, 100M Timed Test
|
Validated Trial Standard (PMID: 38491364) | Every 6–12 Months (Clinical) | The North Star Assessment for Limb-Girdle Type Muscular Dystrophies (NSAD) and timed velocity tests are the primary validated functional endpoints for tracking LGMD R4 progression. |
|
Cardiac MRI, Echo & Troponin
LVEF, T1/ECV, LGE & hs-cTnI / NT-proBNP
|
Standard of Care (Mandatory) | Annually (or every 6m if abnormal) | Up to 50–60% of LGMD R4 patients develop cardiac involvement (dilated cardiomyopathy; Semplicini et al. PMID: 25862795). Cardiac MRI detects subclinical myocardial fibrosis (LGE/ECV) prior to LVEF drop. |
|
Serum Creatine Kinase (CK) & LFTs
Total CK, AST, ALT, GGT, Bilirubin
|
Validated Biomarker | Baseline & Serial Clinical Visits | Reflects acute sarcolemmal leakage (note: CK naturally declines in late-stage disease as muscle mass is lost). GGT and bilirubin are critical for monitoring hepatotoxicity in AAV trials because AST/ALT are confounded by muscle release. |
|
Quantitative Muscle MRI
Dixon Fat Fraction & Water T2
|
Natural History Standard | Annually in Natural History / Trials | Quantifies progressive fibro-fatty replacement of pelvic and thigh muscles and active muscle edema/inflammation with high sensitivity. |
|
Exploratory Fibrotic & Renal Markers
Serum TGF-β1, MMP-9; Urinary Cystatin-C / β2M
|
Exploratory Research Only | Preclinical / Investigational Protocols | Research-stage biomarkers proposed to evaluate anti-fibrotic signaling and proximal renal tubular safety in investigational drug studies. |
Standard of Care Context
Multidisciplinary Supportive Care Standards
Best practices established by international neuromuscular consortia for managing pediatric and adult patients with β-sarcoglycanopathy.
Prescribe structured submaximal concentric exercises and aquatic therapy. Strictly avoid eccentric resistance training (e.g. running downhill, heavy plyometrics) which creates high-tension mechanical shear and tears unprotected lipid bilayers.
β-sarcoglycan is heavily expressed in cardiac tissue. Initiate annual cardiac MRI and Holter monitoring by age 8–10. Early introduction of ACE inhibitors or angiotensin receptor blockers (ARBs) is recommended at the earliest non-invasive sign of myocardial strain.
Serial spirometry (Forced Vital Capacity [FVC] sitting and supine) every 6–12 months. Early identification of nocturnal hypoventilation for timely introduction of non-invasive positive pressure ventilation (NIV), alongside scoliosis surveillance.
