SRP-9003 (Bidridistrogene Xeboparvovec): Phase 1/2 & Phase 3 EMERGENE Expression Data vs. July 2025 FDA Clinical Hold

Systemic IV infusion of bidridistrogene xeboparvovec (SRP-9003) at 7.41 × 10¹³ vg/kg demonstrated strong biological proof-of-concept in both Phase 1/2 (Mendell et al. Nat Med 2024, PMID: 38177855) and Phase 3 EMERGENE (NCT06246513), alongside critical 2025 vector-class regulatory hurdles:

• Phase 3 EMERGENE Biopsies: +43.4% PPF in ambulatory (n=11, P < .001) and +23.9% PPF in non-ambulatory (n=6, P < .02) cohorts at Day 60.
• Phase 1/2 (PMID: 38177855): 36.2%–62.1% normal β-SG expression with motor stability across 2–5 years.
• July 2025 Regulatory Hold: Following AAVrh74 liver toxicity events in Duchenne trials, Sarepta paused its LGMD programs and the FDA placed a clinical hold on investigational LGMD gene therapy trials in July 2025.

CFTR Corrector C17 Rescues Missense β-Sarcoglycan in Primary LGMD2E/R4 Patient Myotubes

Peer-reviewed studies by Prof. Dorianna Sandonà's group (Scano et al. Int J Mol Sci Dec 2024, PMID: 39769077) and Prof. Chris Weihl's group (Li et al. J Clin Invest 2023, PMID: 37317968) have transformed our understanding of missense SGCB variants:

• Deep Mutational Scan (PMID: 37317968): Mapped cell-surface localization across all 6,340 possible SGCB amino acid substitutions, confirming missense trafficking defects correlate with clinical severity.
• Corrector C17 Rescue (PMID: 39769077): Small-molecule CFTR corrector C17 prevented proteasomal ERAD degradation of missense β-SG and restored the full sarcoglycan complex at the membrane in primary patient myotubes.
Stage: Validated in primary human patient myotubes & murine models (Preclinical).

Base Editing in Sarcoglycanopathies: Primary Muscle Stem Cell Proof-of-Concept & Delivery Hurdles

Prof. Simone Spuler's laboratory demonstrated that Adenine Base Editing (ABE) can repair a pathogenic sarcoglycan transition mutation in human primary muscle stem cells with high efficiency and without double-strand DNA breaks (Escobar et al. JCI Insight 2021, PMID: 33848270):

• Theoretical SGCB Application: c.341C>T (p.Ser114Phe) is a transition mutation theoretically amenable to antisense ABE, whereas c.552T>G (p.Tyr184* amber stop TAG) is a transversion requiring Prime Editing.
• Translational Reality: No clinical trial exists yet for base or prime editing in SGCB; systemic non-viral delivery to human skeletal muscle remains the primary bottleneck.
Literature Benchmark: Escobar et al. JCI Insight 2021 (PMID: 33848270).

GRASP-LGMD Consortium: Defining Validated Clinical Endpoints Across Limb-Girdle Muscular Dystrophies

The Genetic Resolution and Assessments Solving Phenotypes in LGMD (GRASP-LGMD) consortium published a multicenter clinical endpoint validation study (Doody, Alfano, Diaz-Manera et al. BMC Neurol 2024, PMID: 38491364) alongside the global JOURNEY natural history study (NCT04475926):

• Functional Standard: Validates the North Star Assessment for Limb-Girdle Type Muscular Dystrophies (NSAD), 10-Meter Walk/Run, and 100-Meter Timed Test.
• Cardiac & Pulmonary Care: Reinforces mandatory annual echocardiogram / cardiac MRI surveillance and proactive ACEi/ARB cardioprotection in LGMD2E/R4.
